Practice Nurse

• Competitive base rates (depending on experience level) plus super and penalties

• A minimum of 4 years’ experience in a relevant clinical environment is highly desirable.
• Bachelor of Nursing or equivalent is a mandatory qualification for this position.
• Possession of a relevant postgraduate or master’s qualification or evidence of enrolment in professional development programs, including experience or study relating to Chronic Disease Management is highly desirable
• Must have experience in General Practice/Primary Health
• Demonstrated advanced clinical assessment, planning, and care delivery skills required
• Experience in chronic disease management, care coordination, and patient caseload oversight required
• Travel and accommodation provided
• Applicants must be registered with AHPRA and hold working rights for Australia
