Continuing Care Stream RN

• Competitive base rates (depending on experience level) plus super and penalties
• Contract available from 16th November to February 2027
• Must have Mental health nursing experience in Continuing care 
• Working with Vulnerable People Registration (TAS) required
• Travel and accommodation provided
• Applicants must be registered with AHPRA and hold working rights for Australia

This Continuing Care role is based at the Huonville outreach clinic, providing mental health support across the Huon and Channel regions through clinic appointments, home visits and community outreach. The role requires autonomous clinical decision-making, care coordination and risk management for consumers with complex needs. Higher-risk home visits are always conducted by two staff members.

Located in Huonville, Tasmania, the facility serves the surrounding Huon and Channel regions, including rural and remote communities. The area offers a regional setting with access to scenic landscapes, local amenities and nearby outdoor attractions.
